Enrollment
Upon acceptance of an offer of enrollment, Discovery Charter School will create an account for the student in PowerSchool and provide instructions on how to submit required documentation electronically in the accompanying Parent Portal. Please have the following ready for submission via the Parent Portal.
The following will be required:
1. Applicant’s Birth Certificate.
2. Proof of Indiana Residency documents. In order to provide proof of residency, the
address needs to be the same on provided documents (one of each type e.g. two utility
bills do not suffice) and also match what is written on Discovery enrollment forms and
applications.
• Copy of current (within 60 days) mortgage statement or lease agreement with the
following:
Parent/guardian name
Address
Date of statement
• Copy of current (within 60 days) utility bill, bank statement, payroll stub or
insurance statement. Acceptable utilities are water, sewer, electric, and gas. Cell
phone and internet statements are not acceptable.
3. Proof of Identity: Non-expired Driver’s License, state-issued ID, or Voter’s registration
Card
4. Copy of current IEP (Individualized Education Plan), BIP (Behavior Intervention
Plan), 504 Plan, and/or other educational plans, if applicable.
Additional paperwork will be required before the school year begins.
